Does anyone have a copy of or know what confidential questions administrators and/or parents are asked to fill out for teachers on Search?

It won't be rocket science. Brainstorm a list of 40 questions you would ask parents about their child's teacher and you can guarantee you've generated most of the list.

Same with admin.

I've seen them before but forget the exact details. It's basically a list of statements about your qualities a teacher/employee and the rater checks off where they rate you. I believe the admin evaluation response is based on top 10% of teachers, top 50% etc. There is a space for comments but it's not required.

Its a link to a survey that asks the rater to rank the IT candidate on a scale of 1-7 on various dimensions that pertain more to employee ability rather than teaching ability. There is a notes section that the rater can make free form comments, but it isnt required.
